I can’t seem to figure out how to get the appointment form set up correctly. I would like to change the wording of Appointment Form to Have Questions?
Secondly, I can’t get the form to email to my email address.
I have tried searching these forums and just am flustered. Sorry.

Kindly go to >> sidebar.php and find and change Appointment Form to what u need.
And change your email id as well.

Kindly go to Appearance >> Widgets >> Sidebar Main, drag and drop a text widget, give it title “Have Questions?” and paste below shortcode in text view then change [email protected] to your email address.

Dave thanks I used your tip about the side bar widget and just went with Ninja Forms… but thank you I think everything is perfect now…
