Inserting Forms Into pages

I have tried to insert forms so that they display on pages but this is all I get. i have tried a few skt themes and it seems the same on all of them. Please help

I am using ninja forms

It seems that one of the plugin conflict with ninja form. Kindly deactivate all the plugins except ninja form and check whether it is working or not.

I decativated ALl plugins except ninja forms. It still does not display correctly in the page,
