Spam from Booking / Appointment – SKT Medical Pro

Hello,

I am using the Medical Pro theme and this has a Contact Enquiry on the sliders, i have removed this by using:

#appointment_main{display:none!important;}

But still getting a lot of spam with the

Subject: Contact enquiry – website url
Name:
EMail:
Phone:
Booking Date:
Message:

How can i prevent this?
